Improving patient safety through feedback on prescribing errors (Imperial College Healthcare NHS Trust, England)

The Prescribing Improvement Model aimed to improve patient safety by reducing prescribing errors made by foundation year 1 doctors in the hospital setting. 

In this project undertaken by Imperial College Healthcare NHS Trust, the "change theory" was that provision of feedback on prescribing errors by pharmacists would facilitate learning, reflection and changes to practice, and thus increase the safety of prescribing.
